📋 Authorization Terms

I, the undersigned parent or legal guardian, hereby grant Community Thrive (a Florida 501(c)(3) non-profit organization) and its representatives permission to:

• Photograph and/or video record my child during Community Thrive programs, events, and activities
• Use these images and recordings for educational, promotional, fundraising, and outreach purposes
• Publish such media on the Community Thrive website, social media channels, newsletters, brochures, presentations, and other promotional materials
• Share approved media with affiliated partners and news outlets

I understand and acknowledge that:

• No financial compensation will be provided for the use of my child's image or likeness
• My child's full name will not be published without separate written consent
• This authorization remains in effect until I revoke it in writing
• I may revoke this authorization at any time by contacting Community Thrive — note that previously distributed materials cannot be recalled
• I am at least 18 years old and the legal parent or guardian of the named child
